Program Details

* These plans are for patients who do not have dental insurance and cannot be used in conjunction with Care Credit
*Valid with health insurance savings accounts, fl ex spending accounts and health reimbursement accounts
*Patient portion of treatment fee is due at the time of service
*Patient is responsible for scheduling and keeping appointments included in the plan within the plan enrollment year
*The type of cleaning appropriate for each patient will be determined during the periodontal evaluation portion of the exam. Patients with bone loss (periodontal disease) will be recommended to enroll in the Perio Cleaning Package.
*Some patients may require scaling and root planing prior to their periodontal maintenance cleanings AND/OR additional cleanings per year to maintain their dental health. These appointments would qualify for a 15% discount under the plan.
